I'm sorry I can't really provide more accurate information, but playing on the FCV server(s) I have noticed that recently there are problems with Mumble from time to time. What happens is that, probably when people switch or get switched, that person ends up in the wrong channel(s).
Two examples:
One is that I had it happen a few times that people who are dead talk to the wrong team on local just after switching. That's maybe to be expected and not a big deal.
Second one was recently on Arctic Lion. I had a guy talking in my squad's channel who wasn't in my squad. After a bit of confusion and talking back and forth, turns out he was in the same squad number on the enemy team (Netherlands). He could talk to us and we to him. Apparently he could also hear his squadmates, but they couldn't hear him talk.
I'll try and see if I can get a more thorough report if something like this happens again.
PS: And a third thing, on one round a few days back, a whole squad I was in could hear the SL radio but not answer. It started suddenly and was over a few seconds later. Can't remember any specifics, sadly.
